Upon the signing of this application. I represent that all of the information now or hereafter given by me in support of my application for employment is true and complete. I authorize you to verify any of the information which I have provided in this application with the appropriate individuals, companies, institutions or agencies and I authorize them to release such information as you require, including my prior disciplinary record, without any obligation to give me written notice of such disclosure. I also authorize you to release any information requested by any of my prospective or subsequent employers without any obligation to give me written notice of such disclosure. I hereby release you and them from any liability whatsoever as a result of any such inquiries and disclosure. I agree that any false information in support of my application may subject me to discharge at any time during the period of employment.


IN CONSIDERATION OF MY EMPLOYMENT, I AGREE THAT MY EMPLOYMENT AND COMPENSATION CAN BE TERMINATED, WITH OR WITHOUT CAUSE, AND WITH OR WITHOUT NOTICE, AT ANY TIME, AT THE OPTION OF EITHER H. J. OLDENKAMP COMPANY OR MYSELF. I UNDERSTAND THAT ONLY AN AGREEMENT IN WRITING SIGNED BY THE PRESIDENT, THE VICE PRESIDENT AND I, CAN MODIFY THE AT-WILL NATURE OF MY EMPLOYMENT. I ALSO AGREE TO CONFORM TO THE RULES AND REGULATIONS OF H. J. OLDENKAMP COMPANY.


I FURTHER AGREE NOT TO COMMENCE ANY ACTION OR SUIT RELATING TO MY EMPLOYMENT WITH H. J. OLDENKAMP COMPANY MORE THAN SIX MONTHS AFTER THE DATE OF TERMINATION OF SUCH EMPLOYMENT AND TO WAIVE ANY STATUTE OF LIMITATIONS TO THE CONTRARY.


I further agree that my employment is conditional upon satisfactory completion of documentation as required by the immigration Reform and Control Act of 1986.
